Access Your W-2 and 1095-C Tax Forms

You must create and activate a secure account using Direct Authentication.
Your W-2 and 1095-C forms are now available online, if you provided consent to receive your forms electronically either last year or by Jan. 8 of this year.
To download your forms:
- Log in to HR/Payroll Connection
- Click “View My Pay” You will need to create and activate a secure account using Direct Authentication, if you haven't already done so. If you have, simply log in to your secure account.
- Click “Access Current Year-End Statements”
- Select your 2016 forms to “Download”
- Review and select “Complete Order”
If you didn’t provide online consent, your forms will be mailed to the address we have for you in HR/Payroll Connection (postmarked by Jan. 31). Your forms will then be made available online beginning Feb. 16 in case of loss. If you have questions, call the HR/Payroll Connection Support Center, 1.844.450.9450, Monday through Friday, 8 a.m.-8 p.m. EST.
NEW Secure Account: All employees are now required to create a secure account using Direct Authentication to access pay and tax information through HR/Payroll Connection. These new security measures will further safeguard your personal information. Direct Authentication requires you to set up your own personal account and password. You'll find step-by-step instructions on HR/Payroll Connection to guide you. As of Jan. 31, you'll need to use your secure account in order to access your pay statements, W-2 form, direct deposit information and other pay or tax information.
Note: During the sign-up process, employees have reported experiencing delays with the email and SMS verifications. To assist in this issue, the timeout window has been temporarily extended for emails/SMS to one hour (from 15 minutes).
