Provide Online Consent by Jan. 7 to Receive Your 2017 W-2 and 1095c Forms

Deadline for online consent is Jan. 7, 2018
By Jan. 31, 2018, 2017 W-2 and 1095c forms will be available via HR/Payroll Connection for all employees who have provided consent to receive their tax forms “online only.” The actual availability date will be communicated by mid-January. Anyone who has previously consented will not have to consent again. However, if you have not consented in the past, and wish to now, you can do so following the directions provided below by Jan. 7, 2018. You will need to consent for both W-2 and 1095c. It is extremely important to be aware of the deadline for providing consent and the availability of your 2017 tax forms. There will be no exceptions to this deadline.
For those of us consenting to online access, we will not receive a paper copy in the mail.
To provide consent, log into HR/Payroll Connection. Click on My Pay, select Electronic W2 Consent. Under Year-End Tax Statements > select Manage Electronic Tax Statement Options to review and make any necessary updates to your information. If you have previously consented, this is also a great time to review and make any necessary updates to your notification options as soon as possible to ensure easy access to your 2017 tax forms.
We encourage all to provide electronic consent. Some of the added benefits of “going paperless” are:
- Ensuring timely retrieval of your 2017 tax forms
- Filing taxes earlier (you may receive refunds sooner!)
- Reduces the chances of a lost or misplaced document
- Helping to save our environment and conserve our natural resources
- The monies saved on postage expense allows CHI to further support our ministry
In order to retrieve your tax forms online, you must have provided an electronic consent by 9 p.m. ET on Jan. 7, 2018.
Declining To Provide Consent
If you do not provide consent on or before Jan. 7, 2018, your 2017 W-2 and 1095c forms will be mailed to your mailing address, identified on the Personal Information > Addresses tab of HR/Payroll Connection. Your tax forms will be mailed by Jan. 31, 2018. Your W-2 and 1095c forms will then be made available online beginning Feb. 9, 2018 to provide you access to reprint a form in case of loss.
