Because your well-being is important to CHI and KentuckyOne, we make sure you and your family have access to comprehensive, cost-effective health coverage. We also work to manage the rising cost of coverage and minimize its impact on you.

One way we manage costs is by re-verifying that any dependents covered by your CHI medical, dental and vision benefits are still in dependent relationships with you. Some relationships – with spouses, legally domiciled adults or step-children – can change over time. CHI will soon conduct a re-verification audit to ensure that your dependent relationships are still in place.

If you’re enrolled in the CHI medical, dental or vision plans and have covered dependents who were last verified three or more years ago, you'll receive a re-verification request in your home mail during the coming weeks. You'll need to take action to ensure that all of your dependents remain covered; the request will include due dates for your responses and examples of the documentation you'll need to provide.

If you don't respond, any dependents who require re-verification will be dropped from coverage and won't be able to rejoin the plans until annual enrollment for the 2019 plan year (unless you have a qualifying life event that enables changes). Note that you won't need to re-verify biological or adopted children – those relationships are not subject to change.
