Good Catch Reporting World Series Final Report

Jewish Hospital and Frazier Rehab Institute created a new program highlighting opportunities for improvement in the delivery of patient care and placing a positive connotation to reporting and investigating “near misses” called The Good Catch Reporting World Series, aligning with the Major League Baseball World Series. Safety Coaches drafted their multi-disciplinary teams and competed against one another in the two month program. Our winning team, The Swiss Cheese Plugs, reported a total of 100 near misses!
The primary goal of this initiative was to increase near miss reporting by 15% compared to the previous year. The program generated almost 500
near miss reports in two months compared to 88 during the same time period last year, which greatly exceeded the goal. Additionally, overall reporting, at our institution reached an all-time high during this time period which is a sign of increased staff engagement in safety efforts. We greatly appreciate all the effort and dedication to keeping our patients safe!
