Have You Shared Your Feedback?
Take the Survey and Win!

As part of creating a strong KentuckyOne Health culture, we are seeking employee feedback through the Performance Culture Assessment (PCA) survey. Administered by HealthStream, an outside vendor, your responses are strictly confidential. KentuckyOne Health does not see individual responses.

Take the brief online survey by April 15.

You'll need your employee ID number, which you can find by logging on to HR/Payroll connection (where you check your pay voucher) on CHI Connect. If you need additional help, ask your supervisor. Be assured, your identity will be kept anonymous by HealthStream.

Help us meet our 80% employee participation goal. As we reach the following participation milestones, random prize drawings by HealthStream will be held:

At 50%: 100 employees will win a $25 Wal-Mart gift card
At 65%: 25 employees will win an iPad2
At 80%: 1 lucky employee will win a $750 travel gift card

Please note: ULH | JGBCC employees will not participate in the PCA in 2013.
