New Tax Form 1095-C:
Completing Your 2015 Taxes

CHI will distribute IRS Form 1095-C to all employees who were enrolled in or required to be offered health insurance coverage for the previous calendar year. This form will contain health insurance coverage information you may need to report as part of your income tax submission for the 2015 tax year and all future years.

Reminder – CHI 401(k)
Annual Increase Program

Remember, participants enrolled in the CHI 401(k) Plan’s Annual Increase Program will have their contribution percentage automatically increased again this year. You can confirm your enrollment in the program by accessing the Fidelity NetBenefits website and selecting “Contribution Amount” under the CHI 401(k) Plan. To make changes, or opt out, you must do so by Feb. 5.

Clean Hands Campaign Kicks Off at University of Louisville Hospital

The Clean Hands campaign launched on Jan. 19 at the University of Louisville Hospital and will run through April 20. Watch the kick-off video! The goal of the campaign is to heighten awareness and increase hand washing practices for patients, visitors, staff and physicians. We hope to have 100 percent participation by the end of the campaign.

One of Kentucky’s former top health officials has joined the leadership team of KentuckyOne Health to coordinate population health initiatives across the system. Stephanie Mayfield Gibson, MD, a board-certified anatomic and clinical pathologist with a 10-year record of public health management, will serve as vice president of population health for the KentuckyOne Health system and chief medical officer for KentuckyOne Health Partners.
