Phase II Cardiac Rehab Moving to Outpatient Care Center

To address new guidelines for Cardiac Rehab programs in regard to patient monitoring, effective Tuesday, July 31, 2018, the monitored Phase II Cardiac Rehab program will be moving from the KentuckyOne Health Healthy Lifestyle Center located in Medical Plaza II to the first floor of the Jewish Hospital Outpatient Care Center (OCC). The new location will be on the first floor, directly down the hall from Outpatient Registration. The Healthy Lifestyle Center will be closed on Friday, July 27 and Cardiac Rehab services will be unavailable on Monday, July 30 to facilitate these changes.
Patients in the Phase II Cardiac Rehab program will receive complimentary parking in the Jewish Hospital parking garage adjacent to the OCC. Valet parking will be free for patients with a valid handicap tag.
Only those patients in Phase II Cardiac Rehab will be moving to the OCC location. All other Healthy Lifestyle Center patients/participants will continue to use the Medical Plaza II location for programs and services. This change though will cause some modification to operating hours.
Beginning Monday, July 30, the Lifestyle Medicine Program will change its operating hours to Monday through Friday, 6:30 a.m. to 10:30 a.m. The staff currently providing both monitored cardiac rehab services and Healthy Lifestyle Center services will remain the same, and is committed to providing the same high level of personal care, education and supervision that our patients and program participants have come to expect.
